When you take several medicines, or look after someone who does, the details pile up quickly: doses, refills, side effects, test results and questions for the next appointment. This binder puts them in one place, with large rows that are easy to read and write in.
The front section holds the information you want on hand in an emergency: personal details, two emergency contacts, insurance, doctors and pharmacy, allergies, conditions and medical history. The medication list records each medicine with its dose, how often you take it, what it's for, who prescribed it and when you started. Vitamins and supplements get their own list.
You can tick doses by time of day, by week or across a 31-day month. There are logs for as-needed medicines, refills, dose changes and side effects, plus symptom, blood pressure, blood sugar and weight logs. The back section has a caregiver log, doctor visit notes, a vaccination record and emergency wallet cards. These pages help you keep records; they don't replace the instructions of your doctor or pharmacist.
